We are proud to announce the launch of OVIK Health, the new brand platform of Milliken Healthcare Products. We value you as a customer and want to make this name change as seamless as possible for you.
Payment Information
- A new W-9 showing the company name will be sent to you. You can also download a W-9 from this page. This new document will show the updated OVIK Health company name, but all other information—address and Federal Employer Identification Number (FEIN), for example—will remain the same.
- Please reference our new payment information documents and note applicable changes.
- If paying by ACH or wire, the “Remit to” company name will NOT change. All ACH/Wire info is located at the bottom of the invoice in RED.
- If paying by check, only the company name will change. The remittance address will not change.
- If you also buy “Andover” products from us, please note that Andover W-9s stay the same. Only Milliken Healthcare W-9s are changing to OVIK Health. Milliken Healthcare is a legal entity changed to OVIK Health. Andover Healthcare is a brand name/dba change (no legal entity change).
